Whether you’re recovering from an illness or learning to deal with a disability, there’s no place like home. Let Dale Medical Center Home Health come to you in your time of need.
We offer skilled nursing services, home health aide services, physical therapy, speech therapy, and medical social worker.

We work closely with patients, their physicians, and caregivers, providing a plan of care specific to the patient’s home health needs. We ensure the confidentiality of all health care plans and medical records. Our home health team consists of licensed health care professionals who offer in-home-oriented treatment alternatives to patients that have recently been released for the hospital, skilled nursing facility and/or rehabilitation facility, have experienced as issues with a current medical condition or have been newly diagnosed and requires disease management education.

Our home health nurses excel in providing and educating our patients and caregivers on wound care, IV drug therapy, foley catheter care and changes, patient safety as well as providing education pertaining to disease processes, disease management and medication management. Our therapy staff provides skilled in-home therapy while developing home exercise programs to promote improved functions and mobility for our patients.

Service Area


Service area for Dale Medicla Center Home Health include Dale, Houston, Barbour, Geneva, Coffee, Pike and Henry counties based on need and geographical location. Following the physician’s referral, Dale Medical Cetner Home Health will contact the patient and develop a plan of individualized care.

Dale Medical Center Home Health accepts Medicare and most insurances.

Home Health


Our Home health teams consist of healthcare professionals offering skilled care at home to homebound patients. The team will work together to assist you living more independently so you can stay in your home. Our services can help you rehabilitate medical events like a stroke or surgery. We can also help you manage chronic conditions such as diabetes, heart disease COPD or arthritis. Some of the medical services may include skilled nursing education, wound care, physical therapy, speech therapy, home health aide services or medical social worker services.
To qualify for Home Health a patient must have as attending physician or primary provider to order home services, the need for part-time or intermittent skilled services such as nursing, speech therapy or physical therapy and you are homebound.

Homebound means:
     * You have trouble leaving the home without help (like using a cane, wheelchair, walker or crutches; special transportation; or help from another person) because of illness or
          injury.
     * Leaving your home isn’t recommended because of your condition
     * You are normally unable to leave your home because it’s a major effort


Plan of Care


Following your physician or primary provider referral, Dale Medical Center Home Health will contact the patient and develop a plan of individualized care. Dale Medical Center Home Health is an accredited Home Health agency covered by Medicare and most insurances.

Dale Medical Center Home Health professionals perform an admission assessment and then develop an individualized plan of care for your specific needs. They regularly review your plan of care and promptly inform the physician or primary provider of any changes in your condition that may call for alterations in the plan of care, with you and your caregivers also being educated about the plan of care.

The nurse will contact you to schedule the first visit, usually within 24 to 48 hours after you are released from the hospital or your physician contacts our office with your need for assistance.

The frequency of home health visits will depend on your doctor’s orders, insurance guidelines and your health status. Our staff is available 24 hours per day, 7 days per week, to meet your needs.
